What is counted

An organization is counted when it delivers something to a person with the intent of extending healthy years, through clinical care, screening, diagnostics, or a structured preventive program, and when it is published, not closed, and not a duplicate listing. The 48 figure for the United States counts every such organization across all four pillars of the healthspan economy.

What is left out, and how much

How is longevity medicine regulated in the United States?

The United States has no federal clinic license and no longevity clinic category. It regulates cell therapies as products at the federal level and splits diagnostic oversight between a device regulator and a laboratory regime. State licensing was checked in three states only.

A human cell or tissue product escapes drug regulation only if it is minimally manipulated, for homologous use, not combined with another article and broadly autologous. Anything outside those criteria is a drug or biologic needing approval, and enforcement discretion ended in 2021. There are no approved exosome products, and the only approved stem cell products are cord-blood derived for blood disorders.

Clinical laboratories are certified under CLIA by the Centers for Medicare and Medicaid Services. The 2024 rule to regulate laboratory-developed tests as devices was vacated in March 2025 and withdrawn in September 2025, leaving such tests overseen chiefly under CLIA.
